Post-Surgical Hand Therapy Timeline: Week by Week

Every operation has its own protocol. The outline below is educational, not a universal schedule. Your surgeon’s written instructions always come first.

Early phase (often days to first few weeks)

Goals usually include edema control, wound protection, and safe motion within limits. Many tendon protocols use a protective orthosis and prescribed exercises several times daily. Skipping early therapy when it is indicated increases adhesion risk.

Intermediate phase

As healing progresses, therapists expand range of motion, introduce light functional tasks, and refine scar care. Strengthening is often delayed until tissues can tolerate load — starting heavy grip work too early is a common setback.

Later phase

Focus shifts to endurance, work simulation, fine motor skill, and return-to-duty testing. Full remodeling of soft tissues continues for months after you feel “mostly better.”

Red flags after surgery

Sudden “pop” with loss of motion, spreading redness, fever, uncontrolled swelling, or colour/temperature change in fingers needs urgent medical review.
